Partnerships - Fay Dental Cares about Kids

Fay Dental Care and Big Brothers Big Sisters of Northeastern Wisconsin have formed a partnership. Fay Dental Care is excited to partner with Big Brothers Big Sisters of Northeastern Wisconsin to provide dental care for any youth “Little” matched in the BBBS Program at no cost to the participant, his/her family or his/her volunteer “Big”.
“Fay Dental Cares about Kids” program is set up to provide dental care for any youth participating in the Big Brothers Big Sisters program. Youth “Littles” can be seen and treated under the same guidelines and protocols that other patients at Fay Dental Care experience with the exceptions listed below:

  1. The program is set up for the volunteer “Big” and only volunteer “Bigs” to bring their youth “Little” to Fay Dental Care’s office. If a parent/guardian brings the youth as opposed to the volunteer “Big”, the youth will not be seen.
  2. Parental/Guardian consent and HIPAA forms must be signed and presented before any treatment. Release form can be found here
  3. Any youth “Little” is eligible for dental care even if the youth “Little” has no dental insurance. If the youth “Little” has dental insurance, Fay Dental Care will submit the dental claim to the insurance, collect the payment from the insurance but not pass a balance or bill for that work on to the participant, family and or volunteer “Big”.
  4. To schedule an appointment at Fay Dental Care, the volunteer “Big” must call two weeks before the desire date and time; if an appointment is available it will be scheduled. Times available for this program will vary. Appointments will be scheduled on a first come first served basis with considerations given to dental emergencies.
  5. If for whatever reason an appointment is missed or reschedule twice, the youth “Little” will not be included in the “Fay Dentfal Cares about Kids” program.
